We are pleased to announce that our family medicine clinic is now offering house call services — bringing quality medical care directly to your home.

What Are House Calls?

House calls allow our providers to visit you in the comfort of your own home for medical evaluation, treatment, and ongoing care. Seeing patients at home also gives our team valuable insight into your living environment, daily routines, medications, nutrition, safety, and caregiver needs — things that are difficult to fully assess in a clinic visit.

Who May Qualify for a House Call?

You may be a good candidate for our house call program if any of the following apply to you:

• You are homebound or have significant difficulty leaving your home due to illness, injury, or disability
• You have limited mobility or rely on assistive devices that make travel to the clinic challenging
• You are recovering from a recent hospitalization or surgery and need follow-up care at home
• You are managing multiple chronic conditions that make clinic visits burdensome
• You are receiving end-of-life or palliative care and prefer to be cared for at home
• You are an older adult who has difficulty with transportation or requires caregiver assistance to travel
• You have cognitive impairment (such as dementia) that makes office visits difficult or distressing

What Services Can Be Provided During a House Call?

• Management of acute illnesses (infections, injuries, symptom flare-ups)
• Chronic disease management (diabetes, heart failure, COPD, and more)
• Medication review and reconciliation
• Post-hospital or post-surgical follow-up and transition of care
• Preventive health assessments and wellness checks
• Home safety evaluations and fall risk assessment
• Nutritional assessment
• Caregiver support and education
• Coordination with home health agencies and specialists
• Advance care planning and palliative care discussions
